In the event of changes in the data, I undertake to immediately inform the financial institution in writing, as well as to provide it with other data and documents, if necessary according to the requirements of the applicable legislation. I declare that I have informed the persons indicated in this document , incl. the beneficial owners and I have their consent to provide their personal data to UBB AD for customer identification and verification purposes, as well as for prevention of fraud and money laundering. I am notified that I should inform them that UBB's Personal Data Processing Information, which describes the purposes and grounds for processing their personal data, is available on UBB's website as well as in each branch of the Bank. I declare that I am familiar with the UBB AD Information for Processing of Personal Data, available on the Bank's website www.ubb.bg and at the bank offices, and through this document I have received information about UBB AD as Administrator of personal details, contact details of the data protection officer, the purposes and reasons for processing my personal data, my personal data categories that the Bank processes, for the recipients of my personal data, my rights in connection to the processing of my personal data and the rights them, as well as the period my personal data are stored within UBB. .I am aware of the penal liability for declaring false circumstances pursuant to Art. 313 of the Penal Code. I declare that I am notified about the possibility the information under Art. 142b of TSSPC containing personal data, account balance or value as well as the derived income to be subject to automatic exchange of financial information under chapter sixteen, section IIIa of the TSSPC and to be reported to the Jurisdiction/s of which the Entity and the controlling persons are tax residents in pursuance of the international commitments of the Republic of Bulgaria. I have been informed that the Bank processes my personal data on the basis and in accordance with the Personal Data Protection Act (EPAA) and Regulation (EC) 2016/679 of the European Parliament and of the Council of 27.04.2016. on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (Single Data Protection Regulation) and that the Bank has taken the necessary technical and organizational measures ensuring effective security and protection of my personal data and my rights as a personal data subject.
